Great Bridge gathering at ASA conference!
On March 25, we happily gathered with many Bridge sites, supporters, and individuals interested in learning more about Bridge, at the American Society on Aging conference.
In addition to sharing updates on Bridge Model growth and developments with the group, we were also lucky to hear from two Bridge replication sites about their successes. Steve Touzell, from Philadelphia Corporation for Aging (the first external replication site trained in Bridge, in 2012) shared how the Bridge Model has helped their organization meet the range of needs of clients they work with in North Philadelphia. We also heard from June Simmons and Jody Dunn of Partners in Care Foundation in California, who spoke about how Bridge has been an important complement to their implementation of the Care Transitions Intervention for the three Community-based Care Transitions Program initiatives that they have – helping them to reach individuals who may be resistant to home visits for various reasons or live beyond geographic reach.
