Monday, March 23, 12:30 – 2:00pmHospital-Community Collaboration: Opportunities and Challenges in the Post-ACA World

Our Bridge Model National Office management team -Walter Rosenberg, Jessica Grabowski, and Bonnie Ewald – will discuss opportunities for collaboration between hospitals and community agencies in meeting the needs of older adults, highlighting successes of Bridge Model sites. Location: Horner (West Tower, Silver Level, 3rd floor)

Thursday, March 26, 9:00-10:00am: The Healthcare Neighborhood: Building Upon Community Assets Through a Public/Private Partnership

Presentation by long-time Bridge sites Aging Care Connections and Rush University Medical Center along with early Bridge funder Community Memorial Foundation. Come learn how they have worked together to build upon their Bridge programs to develop effective community support networks. Location: Water Tower (West Tower, Bronze Level)

Thursday, March 26, 1:00 – 4:30pm: Potential and Strategies for Care Coordination in a “Post-Acute Care” World

The National Coalition on Care Coordination (N3C) is hosting a half-day symposium on different approaches to after-hospital care coordination. Bridge Model National Office is sponsoring this event, and Bridge team members Robyn Golden and Bonnie Ewald will speak at the symposium. Location: Crystal C (West Tower, Green Level)
